Madrigal is a drug company that sells a liver medicine for a serious fatty-liver disease.
Madrigal Pharmaceuticals is a one-product drug company, and that product is a daily pill called Rezdiffra. It treats a liver condition called MASH, which used to be known as NASH. In MASH, fat builds up in the liver and the liver gets inflamed and scarred. You can think of it as the liver version of a clogged, angry filter that slowly stops working. Before Rezdiffra, there was no approved pill for this in the United States, so doctors mostly told patients to lose weight and hope for the best. Madrigal sells the drug to liver specialists, gastroenterologists, and their patients, mostly in the US, and it got full FDA approval in 2025.
Here's how the money works. Rezdiffra is not cheap, and it is used every day, so each patient is a steady stream of revenue for as long as they stay on it. Madrigal reported about $180 million of Rezdiffra sales in 2024, its first year on the market, and the number has been climbing fast as more doctors learn about it and more insurers agree to pay. The company does not pay a dividend and does not buy back shares. Instead, it spends what it earns on the sales force, on more clinical trials, and on expanding into new countries. That is typical for a young biotech that is still building.
The big risks are simple to state. Madrigal is basically a one-trick pony, so everything depends on Rezdiffra. Bigger players like Eli Lilly and Novo Nordisk are testing their own obesity and liver drugs, and any of those could become a competitor. Insurers also push back on pricey new medicines. On the other side, Madrigal has a head start of a few years, which is a real advantage in a market where millions of people in the US alone may have the disease. The company was founded in 2011 and is based in Conshohocken, Pennsylvania. Its entire story now is whether that one pill can become a standard of care.
