United States · NYSE: AGL ·Market cap:

agilon health helps primary care doctors get paid to keep older patients healthy instead of just treating them when they get sick.

agilon health is a company that works behind the scenes of your doctor's office, mostly for older people on Medicare. Here's the problem it solves: normally, a primary care doctor gets paid a small fee for each visit, whether you get better or not. That means a doctor has little financial reason to spend extra time keeping you out of the hospital. agilon flips that. It takes a big lump of money from the government or an insurer to cover a group of patients, pays the doctors a steady salary, and then lets the doctors share in the savings if their patients stay healthier than expected. Think of it as a gym that gets paid for keeping you fit rather than for each time you walk through the door.

The business is basically two things glued together. One part is the money engine, basically a health plan administrator that handles the insurance risk, contracts, and data. The other part is a support system that gives independent doctors the tools, software, and know-how to manage thousands of patients. agilon earns money by keeping total medical costs below the budget it was given, and it splits those savings with the doctors. Its customers are primary care physician groups, and it serves them mostly through Medicare Advantage, a private version of Medicare. Almost all of its revenue comes from this one model.

The company has grown fast by signing up new doctor groups across many states, roughly doubling its size in recent years. But growth has come with pain: medical costs for its patients have run higher than expected, which squeezed profits and hurt the stock badly. agilon has been cutting costs and pushing its partner doctors to manage care harder. It went public in 2020, is based in Austin, Texas, and today its main job is proving this shared-savings idea can actually make money at scale, not just look good on paper.
